Thank you for your interest in registering! We will be notified as soon as you have registered via the website. We will then register you within a few days or you will be put on the waiting list. When the registration has been processed, you will receive a confirmation email (also keep an eye on your spam box!). From that moment on we are your doctor. Then do not forget to unsubscribe from your old GP so that we can receive your medical file.

The following policy applies to registration:
We only register patients who do live in our district but come from a GP from outside the district and therefore do not yet have a GP within the district.
These are postal codes 1051, 1052, 1013, 1015.
You can register as a new patient by filling in the form below. You will receive a confirmation once the registration has been processed. This may take several days. To transfer your file, you give us permission to request your file from your previous GP and you must also give your previous GP permission to transfer the file to us. You will speed up the registration process by doing this right away.
Practical Information
All consultations by patients with Dutch insurance are declared directly to the insurer. You can find the rates on the website of the Dutch Healthcare Authority, the Nza.
